While femoroacetabular impingement (FAI) has been extensively studied, less is known about the long-term effects of global acetabular overcoverage (GO). A recent study done by Dr. Okoroha’s colleagues provide valuable insights into the impact of GO on joint health, patient-reported outcomes, and the likelihood of requiring total hip arthroplasty (THA) in the future.

Patient Demographics:
This study analyzed 38 patients with GO who underwent hip arthroscopy and matched them 1:1 with a control group based on sex, age, body mass index, Tönnis grade, and labral treatment.
Follow-ups:
Patients were evaluated over a minimum of 8 years, with long-term follow-ups extending up to 18 years post-surgery.
Positive Outcomes:
Both groups achieved similar patient-reported outcome measures (PROMs), pain levels, and patient satisfaction. However, the GO cohort exhibited significantly lower baseline joint space width and greater chondrolabral junction breakdown. Over time, the GO group also experienced a greater reduction in THA-free survivorship, decreasing by 13.1% compared to 5.3% in the matched-control group.
Conclusion
For individuals with global overcoverage, hip arthroscopy can provide meaningful symptom relief, but long-term joint health remains a concern. While patient satisfaction and functional outcomes are comparable to those with normal acetabular coverage, the accelerated decline in THA-free survivorship highlights the importance of early diagnosis and appropriate treatment strategies.
